I acknowledge by signing this agreement, that i have given the accurate and full information on the procedure of obtaining a tattoo, and give my full consent on the application of it.
As this procedure involves minor breakage of the skin with a needle, I understand the health risk therefore i am obliged by law to declare any conditions that will affect this procedure.
By ticking the boxes below, I agree:
I understand the tattoo artist may need to touch certain areas of my body to perform the tattoo.
No complaint of sexual harassment can be made against the tattoo artist.

I understand that not taking care of the tattoo will be high risk for infection, fever and damage to the tattoo itself.

I understand that the studio can take photos of the tattoos for social media (Facebook, Instagram, TikTok) and the tattoo studio website.
